The clinical record for home artificial nutrition, enteral and parenteral. Organised by patient and by care pathway, it follows the patient from prescription to home visits and supplies, through to discharge.
Record structure
One patient, one pathway, every section in its place
Each patient has a demographic sheet and the clinical and administrative sections of their pathway, enteral (NED) or parenteral (NPD). A role selector filters the actions available in each section: data entry, signature or read-only.
Opening and demographics
Diagnosis, reference care pathway, referring physician and caregiver. Pathway assigned with the active prescription.
Prescription and formulation
Calorie and protein requirements, access route, infusion duration. Real-time calculations and compatibility checks.
Home visits
Vital signs, bioimpedance, dressings and nutrition tolerance, recorded on a tablet at the patient's home.
Trends over time
More than twenty laboratory parameters, weight and assessment scales with time series and trend charts.
Devices and supplies
Digital supply requests, 28-day renewal cycle with expiry alerts, loan equipment register.
Consents and forms
Pathway forms with signature status, mandatory flags and pre-filled printable preview.
Two pathways
Enteral and parenteral, each with its own tools
Parenteral (NPD)
- Compounded bag builder. Macronutrients, electrolytes, vitamins and trace elements with live calculation of total kcal and calorie-to-nitrogen ratio, compatibility checks and digital signature.
- Vascular access. Management of CVC, PICC and Port-a-Cath with tracked expiry dates and replacements.
- Tolerance assessment with a dedicated sheet for the pathway.
Enteral (NED)
- Introduction plan calculator. Gradual seven-day progression with live totals for kcal, volume and infusion rate.
- Access route. NG tube, PEG, PEG-J, jejunostomy, surgical gastrostomy, PEI and other routes, with replacement history.
- Thickeners, additives and concomitant therapy, with quantity per pack and administration route for each drug.
Fieldwork
Nurses record data at the patient's home, not back at the office
The home visit section is designed for tablets: vital signs, bioimpedance (phase angle, resistance, reactance, weight, fluid balance), interactive pressure ulcer mapping on a body silhouette with staging and dressing sheet, tolerance assessment. Everything feeds straight into the patient trend charts.
The clinical diary gathers home visits, tests, therapy changes, supply requests and signed consents in a single timeline. The calendar separates nursing visits from medical visits, distinguishing first visits from follow-ups.
Essential alerts, not noise
The system flags only what needs action: supply expiry with advance notice, access device expiry, upcoming appointments.
